ADMISSION POLICY Admission will be open to any child, provided the school can meet his or her needs. Students from 3rd grade to 8th grade are admitted on the basis of a placement test and previous school records. Admission to the school will be granted without discrimination in regard to sex, race, creed and religion. All incoming students must submit the following forms before starting school:  Application form  A certified birth certificate  Social Security card  Report Cards from previous school  A signed statement of cooperation  A current updated Florida HRS 680 (immunization record) signed and dated by a Physician  A current Florida HRS 3040 (physical) dated within in the past 12 months  Fees (HRS Forms 680 & 3040 are available at any private physician's office, walk-in-clinic or health department.)
