  • Requirements for Admission •A completed application form with each question answered •All required forms signed by both custodial parents: cooperation and financial •A copy of an official birth certificate •A copy of immunization record - GA Form 3231 •Certificate of ear, eye, and dental examination for K5- 5th grades – GA Form 3300 •Paid admission fee (non-refundable) Admission Fee will be returned if all requirements are not met. •If student is new to the school (K5-5th) – copy of transcripts, standardized tests, recommendation form, discipline records, last report card, interview, and admission testing ($45-$50 non-refundable fee applies for admission testing) •Copy of custody papers if issued by court •Potential 1st-5th graders must score 55 or above on SAT (Stanford Achievement Test). If transferring from another school, scores must be provided prior to acceptance.
