General Information For All Applicants By January 31 Completed online Application via the FACTS website (and the $65 application fee per child) Birth Certificate, Baptismal Certificate (for Catholics only), and required Teacher Recommendation from current school/program required. This form is available on the FACTS website.Transcript/report card and standardized test scores required for the current school year and the 2 previous school years. May upload via FACTS or send directly to OLV School.Classroom visit for all applying students (will be scheduled upon submission of an application) By the Second Week of March letters of notification will be e-mailed or earlier if spaces are available. Siblings, parishioners, and Catholic students will be given first consideration for available spaces.
