New Millennium Academy and I/we do hereby enter into a contract with the following understanding:  The administration has full responsibility for placing our son/daughter at the proper academic level. We agree to pay all fees and tuition for each school month on a timely basis (The 1st of Every Month Tuition is Due). If tuition is received after three (3) days of non-payment a Late Fee of $50.00 is applied to the tuition amount due. Furthermore, we understand that being delinquent in fees and/or tuition is sufficient grounds for having our son or daughter released from the school. After release or withdrawal, no transcript or report card will be issued until money due to the academy has been paid. If a family chooses to withdraw before the end of the year, parents will be responsible to pay the remainder of that month of withdrawing. New Millennium Academy reserves the right to administratively release a student if the parent or student does not cooperate with the academy or the educational process. We agree that this decision can be made at the sole discretion of the administration. This contract may not be voided except by action of the Executive Director.  We have read the contract carefully and hereby agree to its terms. A prerequisite for a student`s acceptance into New Millennium Academy is no drug possession charge, no weapons charge, nor an assault charge. No criminal charges of any kind. I/We agree that if my/our son or daughter is released or dismissed for any reason that all fees and tuition payments are non-refundable. I/We hereby have read the Agreements of Discipline, Attendance, and Tuition and Partnership and consent to submit to the policies and procedures outlined herein.   I hereby apply for enrollment at New Millennium Academy. If accepted, I will cooperate with the spirit and regulations of the school. I will cheerfully maintain prescribed standards of dress, and conduct, including the wearing of proper uniforms and total abstention from the use or possession of tobacco, drugs, drug paraphernalia, alcohol, sexual misconduct, or assault. I will never bring weapons on campus under any circumstance. In signing this application, I am giving the academy assurance that I understand I can be removed from the academy at any time if I fail to abide by these requirements.Parent/Guardian PledgeI attest that the information provided in this enrollment application is true and accurate. I understand that if any information is found to be false or misleading, the application process may be terminated or the applicant may be dismissed from New Millennium Academy. I also understand that acceptance is contingent upon the applicant`s satisfactory completion of his or her current school year, both academically and behaviorally. I also understand that my son or daughter may be released from the school for not adhering to the school`s conduct requirements.
